Custome Form Adding Items Manually to combo box not working

I have created a custom form for an existing SharePoint List. In the form people need to be able to add new items to the drop down. It works on the Default Form but Does not in the custom form. 
 
I added this code to Update filed of the Card
 
When I go to test it, I get this error. It is like it doesn't think anything is in it.

    Remove the required field validation and create a custom validation in the button click

    You have created the button for submit / save right. 
     
    In the button you can create the validation.
