PowerApp Form not Including Entry Boxes

I'm currently working on a PowerApp that enables data write back capabilities to the underlying SQL table. I created a PBI report, created a PowerApp, and did some simple formatting. In order to enable write back capabilities, I added the underlying SQL table to the PowerApp and then I created a new form screen in PowerApps. I connected this form to the SQL table, and once I did this, nothing showed up in the form and I had to manually add the fields in. The issue I'm having is when I add these fields manually to the PowerApp form, there is no "entry box" for each field. When I worked on a similar project to this a couple months ago, once I connected the PowerApp form to the target SQL table, this form automatically was populated with the fields as well as entry boxes. I did essentially the same steps, but I'm not seeing these entry boxes in my current report and it's preventing me from implementing CRUD operations in the PowerApp. From my previous experiences with PowerApps, forms are usually automatically populated with entry boxes when connecting to a data source. The first two screenshots show how I manually added the fields into the PowerApp form where there are no entry boxes. The last screenshot is someone else's project, but it is essentially the type of form I need (fields with entry boxes). If anyone knows what the issue is and how to solve this, please let me know.     The problem may be with the connection to the SQL table.
     
    What type of connector are you using?
    Is the connection actually working?
    Do you have permissions to view the table?
    What permission does the user account that you used to create the connection have?

    Can you do something for me.
     
    1. Question: do you have a key on this table?
    2. Can you delete that form and any reference to the table, then go to your DataSources, select it and do 2 things
    a) Refresh it(via ... Refresh)
    b) Edit the Data
     
    See if b actually brings up a table that shows the data that you can edit.
     
    If not then we will have to look at the structure of your table (width, indexes etc that can conflict with it not giving you fields to edit) essentially not having permission and or metadata block

    Thanks for your reply. I do have a key for this table, which is the ID column. And I was able to go to Data Sources and Refresh it and Edit the Data. So maybe there is something else that is going wrong. The weird part is that I actually created a PowerApp with no issues using this same data source a couple weeks ago, but I accidentally overwrote it and it got deleted. Now I'm running into this issue that I didn't face before.

    Thanks for your reply. The connector I'm using is a SQL Server connection (on-premise gateway). And yes, the connection is working I think. And I confirmed with my IT admin that I have viewing and editing permissions on this table. As I said before, what's really weird is that I actually created a PowerApp with no issues using this same data source a couple weeks ago, but I accidentally overwrote it and it got deleted. Now I'm running into this issue that I didn't face before.

    Try creating a new app. Add the data source and a form and indicate if it shows the fields.
