Employee Engagement Survey

I have several situations in my application, the application is an Employee Engagement Survey and I have the following conditions: in the second screen question number 18. Manager's comments should only be available to the manager of the employee filling out the survey.

In this section the manager is supposed to coordinate a date to follow up the survey with his employee, it is going to require the signature of the employee and the manager. I added the lookup of the manager and the director to use the information in the flow to request the director's signature from the manager and once the signature is obtained to submit the form to the HR department. If there is a more efficient way, please let me know, I accept suggestions.

I have looked for several examples of multi-level approval but nonrelated or similar to an employee evaluation, if someone can help me, I would appreciate it, this is a bit complicated for me. Thanks in advance 🙂
