How to link a group of names to case on a form

Hi All

 

I have a form I have built for COVID-19,when an Employee tests positive for COVID-19 I want to be able to link all the people He/She was  close to using  the case number for example case-001 that has been logged for this person that has tested positive .

This can be up to 30 people ,I want to also be able to also create new cases for those individuals using  that similar form. It is a sql table I have created and the form for contacts is called close contacts .The Close Contacts are our  Employees in a table called Employees.

 

Any ideas how can build this ?

    Modeling human relationships can be much more complex that it appears at first and is more of an art than a science. In your case, some of the contacts may be employees too.
