Trigger a task from a YesNo toggle component in a form

Very obvious newbie here.  

Working in Sales Professional - CRM

I am trying to trigger (create an event??) after a toggle is changed to yes which will then create a task with a defined subject heading and auto populated fields to a specific team member.  

For example

Design Request Yes/No

If yes

- a task form will appear with Design Request in the Subject line

- design description will auto populate with details previously completed elsewhere

Task sent to Team member A only.

I have tried various ways, but nothing works, and I feel like this should be simple.  Any ideas?
