Error submitting form

when I submit form, I get an error that "there was a problem saving data - datasource maybe invalid".

 

I refreshed the datasource but it still does not help.

 

if I pass those parameters to the db directly - the

    What is the formula that you use to submit the form? Please share. What is your data source? How did you create this app?
