What is the difference between 'Site Map' and 'Model-Driven App'?

You can think of a "Model driven app" as the app that users open and run. It's effectively a container that enables users to open/view/edit records from tables that you define.
A "site map" is the definition of the menu items that are accessible from a given "model driven app".
In your screenshot, the name of your "model driven app" and "site map" are both "model driven app", so I understand the confusion here.
The Site Map is an element used internally in the navigation of Model Driven App (the stuff you usually see on the left side while in the model driven app)
The Model Driven App is the app itself.
