Successful form submission not triggering Microsoft Flow

Hello,

I have an app with an edit form that writes to a Dataverse table. The form has an OnSuccess parameter that launches a Microsoft Flow to process the submission into Salesforce as well. This app works 99.9% of the time, but I have had two instances recently where the user submits, and the flow never kicks off. The entry is successful because I see it in Dataverse, but when I go to check on the flow in power automate, there is no corresponding flow run for that entry.

The users did not report receiving any error messages or abnormal behavior. The last part of the OnSuccess parameter navigates them back to the homescreen, and both reported that happened.

One thing I am going to try and develop is a "re-try" flow that runs once an hour to try and re-process entries that haven't made their way into Salesforce yet. Has anyone experienced anything like this? If so, how did you address it?

Thanks!
