Custom Connector (Azure Table storage API) link with Form(Canvas app)

Hi,

I am trying to write the "Form" inputs to Azure Table Storage tables. So i have created a custom connector, makes Post API Call.

But not sure how to integrate the Custom connector with Submit button to call the API.

Can i get some help.
