End of Conversation Survey Not Working

We have built a Chatbot and deployed to MS Teams and generally it is working well.

However, the 'End of Conversation' part does not work when we post to the Team. When testing, it does work OK. See screenshots. We got the 'End of Conversation' from the pre-made bot available through Power Virtual Agents. What do we need to do to get it to work?

I'm not sure this works in Teams - at least it never has for me, so I've made my own version 😄

Thanks. Any guidelines for making my own version? How to I get the stars?
OK, edit, I realised I can just copy and past 'stars' to get the stars.
