sharepoint list not showing fields correctly in powerapps

I have a sharepoint list. That displays correctly in sharepoint.

I have the excel file attached i imported into sharepoint.

 

 

I have tried everything, to fix the issue, but the fields will not show up correctly in Powerapps.
