Gallery items disappearing when I preview the form

Hello, I'm working on a Visitor Sign-in form using Power Apps.  I've created a phone-sized canvas, created a gallery which I've linked to a SharePoint List Datasource, and have populated the gallery with fields for visitors to enter.  After entering their information, they hit a button at the bottom of the page which I've linked to SharePoint using the "patch" command.  This was working well, and the form was adding new entries into the SharePoint List just great.  Then suddenly, while I was working on getting the radio buttons working with the list, every time I hit Preview or Alt or Publish the app, the gallery and its contents are completely blank.  The visibility is turned on, I've tried multiple browsers, I've rebooted...nothing allows the gallery to remain visible during preview.  Any ideas what this could be?

    Did you change the Default Mode control from "New" to "View"? View loads data into a form, so if you are trying to submit new data, the control needs to be set to "New"

    Unfortunately, I don't see that option.  The only thing about the "Visible" and "Position" parameters is "Layout," which is currently set to Blank:

    Apologies, I thought you were using a Form and not a Gallery. I could be wrong, but I believe you want to use an "Edit Form" to have people submit information, not a Gallery. A gallery should only be for viewing data in your data source.

    Oh interesting.  I will look into the form.  I never knew that as an option.  My main frustration is that this was working great and then at some point, Poof, it wasn't working.  Or rather, the gallery was gone whenever I previewed it.  

     

    So does an Edit Form make sense for what I want to do here?  Pass information from the form to populate new lines in a SharePoint List?  I quickly created an Edit Form here and when I connected it to the Datasource, it immediately filled in with the labeled columns from the SP List.

    @CeneroSanchez 

    Yes, you can edit the fields to a degree (like include/exclude, or redesign the look of the form) but when you type into the fields on that form, it should update your SharePoint site with what you entered. You need to have a button with the OnSelect property set to "SubmitForm('formName');" for it to update the SharePoint site.
