Looks like that is far less UI customization available when using the 'Create and app' function from a SharePoint Online list (e.g. I cannot set a custom screen size). But beyond that, is there any functional difference? I noticed that I can "save" the app locally but I don't know if provides any real benefit since (from what I understand) I cannot transfer it to another list in SPO.

The export and import app feature in PowerApps can only be used with apps in PowerApps site that contain the standalone apps list.

You are correct in that you can't transfer apps that are created with "Create an app" to another list in SPO. A big functional difference is that when you create an app using the function in the SPO list, that app is not available in the PowerApps app gallery. That means that you cannot open the app from the PowerApps application on your phone or tablet. You can only access that app from the SharePoint list you created it on. Additionally, you cannot embed that app in a page using the new PowerApps web part.
