No Trigger System Response

Hi there; this is a two-part question.

 

1. When a question fails (ie there is no trigger), where do I update the default system response text?

2. I would like to send myself the failed question so that I'm able to add it as a new trigger.  Is there documentation that would illustrate how/where to do that?
