Embedding model driven apps in teams is not an officially supported thing yet. You can use a website control in your team channel and it will show the mode driven app in your team but it isn’t optimal because there is too much chrome and menu structure doesn’t look great in teams. They announced enhancements to model driven apps in teams at Build so expect this to get more capable over the next year

The official teams integration others posted only works with d365 not general model driven apps
