Submitting New Item also updates a previously submitted item by the same user

I have created an app that allows users to submit new items and documents to a SharePoint backend, and update existing items, if needed. I have a flow to upload documents attached to the submission that is triggered on the "OnSuccess" property of the form, and uses LastSubmit to tag the documents to the correct submissions.

 

Error: When a User submits a new item in the app, it sometimes (no apparent pattern) also re-submits an item that was previously submitted by the same User. The incorrectly updated item doesn’t necessarily have to be the last item submitted by the User. For example, the same User submitted Item #2766 at 8:03AM yesterday and somehow the status and Submission Date for #2695 was also changed at 8:03AM yesterday. The documents uploaded with #2766 are also uploaded again for #2695 and tagged to #2695.

This also happens multiple times to the same item if the author submits a couple of items back to back. The incorrectly updated item is not necessarily the last item submitted by the user.

 

My initial theory was that there is some sort of weird “holdover” effect where the app “remembers” what ID was last submitted and re-submits that, and triggers all the flows as if it were a new item.

  1. I checked if using an item specific link (say for item X) to access the app, and then submitting a new item also re-submits item X but doesn’t look that way
  2. I checked if viewing item X, and then submitting a new item also resubmits item X, but no again
  3. I also checked if keeping item x open in another tab causes the issue but it doesn’t

 

Has anyone encountered this issue? It's really frustrating as I am not able to recreate it on my end and have no idea what could be causing it. Any help is really appreciated!
