Good afternoon,
we have recently implemented Share Point list form (Power Apps) to automate internal approval process. After receiving of a notification via Teams an approver is able to access SharePoint list form via link and check the request details. From time to time an approver sees data from a different previously submitted request instead of the actual data related to submitted list item. At the same time other participants and initiator can see proper data while accessing the same list item.

thanks for your reply. Yes, when initiator raises request the link to PowerApps form is being sent using Flow. When an approver reacts initiator is notified and the link is shared again. You can see form screenshot attached
It seems the issue is with link generation. Could you please compare both the links send out to approver and initiator and confirm the same ID value is there in both the links ?

please let me expose some details. It is not related to the link generation because even if approver is accessing the form from the list the form can some times display attachments of the first list item instead of the attachments related to the current list item. It is not happening all the time and list page refresh solves the issue. However approvers get confused and sometimes reject requests thinking that wrong attachments have been applied by initiator. An important point is that wrong attachments which appear in the form are always from the very first list item.
We have already tried recommendation below but it didn't fix in our case:
