I'm attempting to create a new Data on-premises connection. The Data source type I'm selecting says "Premium" next to it in a rectangular box. What does this mean?

A premium data source is one that requires a premium Power Apps license plan.
Effectively, this means that all users that connect to the on premises data source requires a per-app or per-user license at $10/$40 per user/month respectively.
