Creating an additonal form when an item is selected in sharepoint

Hello,

 

  I'm new to powerapps so forgive my ignorance with this question, but how do I create a custom form when items (it's a checkbox list) are selected in my sharepoint form.  So basically I have built a new list, when a user goes to create a new item, they have boxes that they fill in.  One of those options is a drop down check list where they can select multiple options.  I need to create a custom form when any particular 4 of those choices are chosen.  Or I can create a custom form for each individual choice if needed.

 

  I also have a word doc with the form built already, so I could attach the form instead of building it in powerapps, but how do I do that using powerapps?

 

  Again, I'm new to this, so I need step by step instructions.

    So what if instead of creating a form when the select certain options you do it a little different?

     

    How about have one form with all of the fields? Then hide all of the fields you don't want to show using the Visible property and an If function. Then If the value of the checkbox changes show the fields you want the user to fill out? 

     

    Something like that work?

    This video will walk you through using Conditional Formatting with PowerShell. Basically, on each field, you need to set the Visible property to something like If(Field1.text = "Cow", true, false) This will then only show your field if the value in Field1 is set to Cow. If you use dropdowns or other control types the syntax might be slightly different but the logic is 100% the same. The video will show you the concept in action.
