Form Disappears when the App is Previewed

I'm having an issue with my Form (called Form2, because I'm not very creative with names), that is connected to a data source (Sharepoint), and works how I expect when I'm editing the app:

However, when I click "Preview the App", it changes to this:

Does anyone know why this happens? I can't figure this out.

    Hi @koontz 

     

    Please change the default mode to New:

    With the Edit option the form expects a record to edit. That's why you get the message 'No item to display'.
