Form field datetime split in 2 fields

I've added the Created On field to my form and for some reason it is displaying in 2 separate columns Date | Time. This is the OOTB Feedback entity and there are no scripts or rules on the form.
 
How can I fix this? It is the same whether using New look or not. I've removed/published/re-added but it still shows separate instead of one field like Closed on.

    it is related to the dateandtime field type
    there you have two options:
    - date only
    - date and time
     
    if you select date only you'll get one column
    date and time gives you two columns

    You'd need to write the timestamp to a variable, then use a TextBox to display the value, which is simple for only displaying.  
     
    This is pain if you desire the user to enter/change the value, as you'll need to validate that they've entered a valid date and time. You'd have to do something like this in the Validation:
     
    IfError(DateTimeValue(txtCreatedOn.Text) ...

    It turns out this is how it works for fields on forms. I must have been comparing it with other Date (only) fields.
