Problem with a form that displays in the appdesigner, but isn't in the apps nav menu and is not accessible in play mode or when running the app in the Power Apps application. I have spent the day searching for answers, watching videos, trying every suggestion.
 
I created the form by open the solution, opening the app, navigating to athe table, creating the new form, setint the form properties, e.g. the security to everyone, form order to be first, saving and publishing, publishing all customizations, and so fort. The form looks great in the app designer, but the entry in the nav pane of the app is still the default grid for the table.

    If a form looks fine in App Designer but doesn’t show in the app menu or at runtime, a few things need to be checked:
     
    #1. Navigation (site map) controls what users see, so you may need to add your table or form there.
    #2. Make sure your form is set as the main form if you want it to open by default.
    #3. Check security roles to confirm it’s visible to everyone.
    #4. After publishing, sometimes you need to refresh or reopen the app for changes to appear.

    May i know what is the TYPE OF FORM which you have created?
    If you have created as MAIN (Information) then did you added in Model Driven App ?
    If yes do you have any security Role Associated to form ?

    If all good then open an existing record and check you can have an option to change the FORM
