Create reports in model-driven app not working...?

I am developing a model-driven Power App. We are using managed environments, so there is a version (unmanaged) in a Dev environment and a Managed version in the Test environment. 
 
In both environments, I have System Administrator role. In the Dev version, I am able to create reports in the model-driven app by using the Reports Wizard and this works fine.
 
However, in the Test version, no matter what I try in the Report Wizard, it always fails on the last step with a generic "An error occured. Contact your D365 admin" error message. Can't find anything about this online etc.   I have checked again and again and, yes, the System Administrator role has not been modified so has the permissions to write to the Reports entity and the Publish Org Reports permission etc.
 
Any ideas what I'm doing wrong?   Is it because it's Managed? If so, I don't see how this should be a blocker, as a report using the report wizard should be essentially just creating a new record on the Report table...???

    I believe this is because of Managed Environments, but not because the solution is managed. It’s because Managed Environments can block unmanaged customizations and Report Wizard reports are always unmanaged.
