Permission Issues when entering data via forms

Hi

I'm currently trying to create a very simple booking page in which the user has a form they fill in and it saves it to a table. However, I'm getting the same error regardless of what table permissions I have set up:

Please can someone let me know if there's a step or configuration I'm missing somewhere?

Your services table permission entry does not have Append and Append to permissions. This is required to set the lookup on the Bookings entity form along with the Read permission

As per the previous poster, it is most likely that one or more of the permissions for your Lookups is not correct. To link records via a lookup field there needs to be a Append permission (on the table that you are selecting records in the Lookup) and an Append To permission (on the Table that has the lookup)
