Issues with PowerAutomate Create File in SharePoint

I was attempting to build a solution based on this tutorial:

It is using Quick Parts in Word to create a document from a Power Apps form with a Power Automate instant flow. 
I had it working but now suddenly the files are showing up blank. There is not even a document. The file is there, the metadata for the file is there. But when the file is clicked to open it, it just opens an empty Word window. I can create a new file by clicking the "New" on the SP Doc Library and selecting the content type with the correct form (there's only one content type in the library currently) Since the metadata is all there, it would seem the workflow is working, but why is there no document?
