approval error in 2nd approver

I set up 2 managers for approval flow, the 1st one is no problem but while 2nd "approve" the request, then the flow will automatically turn into "reject", anyone can advise me how to adjust the flow? thanks a lot

    Could you provide a screenshot of your flow?  We need to see the relationship between the two approvers and the approval actions.
