Annual leave approval process using Forms

I want to add a tab in Teams called 'Annual Leave' where colleagues can go in an submit a request for annual leave. There are various line managers within the company. Is there a way to create a flow that looks at the response and sends a notification to the specific line manager of that employee that they've submitted an annual leave request?

Is there then a way that the line manager can 'approve it'?
