Company audit - Sharepoint

Good afternoon, 
I have a project which in a sentence is "build an audit"
I am trying to find the best way to do this
 
We run a user appropiateness audit once a year for about 2000 employees. They are split into departments and it is the responsibility of the department heads to log in, see the names under their department and either confirm or request their user access is modified. This information is captured within the SP list.
 
Today, I tried to build a Sharepoint list and build a flow around when an item has been modified or updated. The issues I am hitting is 
 
a) 2000 lines
B) HOD edits line - add name and time into HOD columns
c) If modifications are required a member of the IT department will then go in and modify the item confirming the changes have been actioned. WITHOUT Overwriting the HOD time and date.
d) is a sharepoint list the best way to go about this, or is there a better platform?
