Accessibility issue Form and Gallery

hi 

I completed building a form and it was fine, but when I started to use it., it shows accessibility issues
And it went off?

But the fields are there when I click the data card, sometime the screen just show empty form?

I don't know how to fix it..   Please help

    Hi @bbsin ,

     

     Once you have tested and submitted a New form, you need it to be changed to Edit mode with an item to display or stay still in New mode for further submission. Do you have NewForm(FormName) on the Submit button?

    the form fields are back in order, but when I click on the form preview, it not displaying now,

    Frankly, I not sure why the font are so small. look like it back to normal now

    Could you please share more details about your scenario? Why you make the form so small, without displaying other Data Cards?
