Adaptive Card: Unable to reach app. Please try again.

Hello,

i know this might have been answered several times, but I cannot find a solution for my flow.

I have created a flow, which is triggered after the submission of a MS Forms document.
I started with a standard adaptive card for the approval, it was working, but it didn't contain all information from my form I needed. Combining functions in Power Automate UI was not possible. So I decided to use the JSON from the attachment.
A screenshot of the start of paarts of the flow is also included.
My flow runs without problem to "Wait for an approval" and when I click Reject or Approve, provide a comment and click submit, I get the error "Unable to reach app. Please try again." on th ecard in teams.

Do you guys have any idea why?
