When you try to uninstall a managed solution that contains any table that has been modified in the action bar (no matter if it's in the grid or the main form), you get a reference constraint error on a system table.
Even if you try to manually remove the Command Library generated by modifying the toolbar, manually remove dependencies, etc. No matter what you try, it's impossible to uninstall the solution.
Even if you modify the content of the solution zip file to remove the xml nodes that reference the "appactions" and try to import the modified solution by hand, you still get the same error.
The conclusion is that you have a managed solution that you can't uninstall and that you can't update either if you remove the modifications made in the toolbar of any table from the modern experience.

Use XrmToolBox tools like Ribbon Workbench to address issues.
Ribbon Workbench: Use the Ribbon Workbench plugin in the XrmToolBox to manually edit and reset ribbon definitions. Solution Dependency Viewer: Identify dependencies more effectively, especially those linked to system tables.
