Store Microsoft Forms responses in a SQL Server table Connection not found

Hello, I am new to MS Form and try to setup the automating process for Microsoft Forms responses in a SQL Server table. However, the MS form has been setup, but the SQL server side keep getting error [ Connection not found. Please create a new connection and change your application to use the new connection.]. I check with my firewall, and sql configure manager. Can someone help me out with this? Maybe need to setup another MS server setting allow it to get through? thanks,
