Filtering lookup

Hi the community,

I have a little question. 

 

Is it possible to filter the result of a lookup column on what has been input on others columns ?

Let me explain

 

I have a form with 2 input fied.

* First field is : you client ID,

* second field is also an input field ; your mail

 

The third field will be a lookup column that shows only the record if field 1 and field 2 is matching.

    Oh Thank you @OliverRodrigues. I'm sure JS will give me some headache because i'm not a developer but if you say i can achieve it with some js like you, i will definitely try this.
