How to send an email after someone submits a form in power pages?

Hi all, I have setup a multiform that populates into a model driven app after the user submits information. I am trying to create a flow that will send an email to someone once the new entry is created in Dataverse but for some reason my flow is not working. I have made sure that it is on. Below is how I have it setup. Any ideas how to get it to work?. Thank you.

Can you check the flow status? Is it throwing an error? Can you post a screenshot of what is happening? Good to see the flow run history.
