The simple way to fix your problem is as follows
1. Open the form in the editor
2. Delete the subgrid > save and publish the form

In Design Studio
1. Pres sync configuration and preview the website. And press "Ctrl+F5"
Make sure you don't see the subgrid now.
1. Now edit the form again > add the subgrid
2. Save and publish the form with the subgrid
3. In the Design Studio, Pres sync configuration and preview the website. And press "Ctrl+F5"
