Form Recognizer Connector Suddenly Failing with 404 "Resource Not Found" Error

Hello everyone,

I have a Power Automate flow that utilizes the Form Recognizer Connector to process documents. This flow has been running smoothly for several months without any issues.

However, all of a sudden, and without any changes made to the flow or the environment, the connector started failing. It returned the following error message:

Unfortunately, I was away at the time and couldn't address the issue immediately, resulting in over 100 flow executions failing during this period.

To resolve the problem, I simply created a new Connection Reference to the same Form Recognizer endpoint, and the flow started working correctly again.

My questions are:

  1. What could cause an existing connection to suddenly stop working without any changes made to the flow or the environment?

  2. How can I prevent this issue from occurring in the future to avoid failed flow executions?

Any insights or suggestions would be greatly appreciated.

    Hello everyone,

    It's been a week since I first posted about this issue, and I haven't received any responses yet. I wanted to provide an update in hopes of shedding more light on the problem.

    Since my original post, this issue has occurred two more times:

    • Once in the same application, where the Form Recognizer Connector again failed with the same 404 "Resource not found" error.
    • Once in a different application within another environment, experiencing the identical error and behavior.

    In both cases, I was able to temporarily fix the issue by creating a new Connection Reference to the same Form Recognizer endpoint. However, this is not a sustainable solution, as the problem keeps recurring and causes multiple flow executions to fail until I intervene.

    I'm reiterating my questions and seeking further assistance:

    1. What could be causing existing connections to the Form Recognizer Connector to suddenly stop working without any changes to the flow or environment?

    2. What steps can I take to prevent this issue from happening again in the future to avoid failed flow executions and the need for manual intervention?

    3. Is this a known Microsoft issue with the Form Recognizer Connector or Power Automate connections in general?

    If anyone has experienced similar issues or has insights into why this might be happening, your guidance would be greatly appreciated.

    Hello there.
     
    Also having the same issue with the Forms Recogniser action. 
    I had replaced the API connection key and it had worked for a couple of runs. But now failing again.
     
    Unsure what to look at?
     
    Could anyone confirm if a new action has been published, maybe a new API version and PA hasn't been updated.
