Populate Portals Form Based on List Item Chosen

I have a Portals page with a list and a form.

Action Desired: Clicking the circled icon populates a form based on the

same table that is populating the list shown. The form would show only

data associated with the line item where the button is clicked (UniqueID

being the 'Concessions Contractor Number').

Yes.

Ultimately, I'd like an image control there as a default that gets covered by a Form and data whenever the icon is clicked in the adjacent list.
