Multiple Child Permissions on same table

Hi Guys,
 
I have one issue going on with Power Pages. Please refer the below table permissions.
 
 
I have three child table permissions of Document entity to show subgrids in Account, Case, Component entity forms. 
When i deactivate any one of the subgrid's child table permission(like in above screenshot: Account>Document is deactivated), i am able to see other two's subrgid perfectly, no error.
But when all three are active, all subgrids of Document entity on the portal showing error, error is also not there, only a red warning sign. Please see below picture.
 
I tried recreating the permissions but still it did not resolve. I also tried other relationship in Document table permission but still error is there.
 
Update : When i remove read access of inactive table permission and activate it again, the error goes and I'm able to create the record in subgrid but the records are not visible as i removed read access. So there is some problem with read access in this scope.
 
Any suggestions?

    Can you please like re-write your ask in like steps, not a paragraph, so we can get the context. Also break it down by entity.
     
    Its a little confusing to follow, so if you can specifically give a detailed data walkthrough with pictures, give the entities fake names doesnt matter, but visualize it for us as much as you can.
     
    The picture doesn't say anything so can't really speak to it.

    Instead of deactivating the permission that you don't want, can you remove it entirely? it might be better for troubleshooting
     
    So far from what I see, your permissions look correct
     
    Can you go through this article and check error log options, maybe this will give the real error message behind it
