I have a simular case on my tenant.
For each approval request, 1 mail from flow-noreply@microsoft.com and 2 mails from maccount@microsoft.com are sent to each approver. If the user approves, the reply is not registered in the workflow.
In the interface to design the workflow, some interfaces are changed. I assume there was a update of Power Automate recently which causes this kind of errors.
