Upload a picture in power portals

Hey, I was trying to upload image in power portal form but the field is not showing. Is there any possible way to upload a picture in power portal form? If I use the note functionality, Is it possible that when it is accessed from model driven the picture automatically showed up without the user having to click or download it? Thank you!
