I am looking for a way to record both Form questions and Answers into a SharePoint list, is this possible?

I have a SP list with columns for FORM questions as ex. Q1, Q2, Q3 and also have columns for FORM answers as ex. A1, A2, A3. The answers are an easy flow choice.

I want to change the FORM questions and record these questions in the specified SP static columns along with any answers in my SP list when the FORM is submitted.

I can't find anything that addresses this process. Thanks!

Thank you for the clarification. Since the get responses action only returns the responses and not the value of the question, you will need to do what I outlined in my original response; you will need to hard code the question into the fields highlighted in your flow.jpg image. The Get response details only includes the unique identifiers of the questions. For example, r917f27f11e4b433c93bfad308c133b94 instead of the actual question.

David, I apologize for the lack of clarity. Yes I am talking about Microsoft Forms. The questions are fixed but I want to export those questions into my SharePoint list along with the answers when the Microsoft Form is submitted. I have attached pictures of my Power Automate Flow, my Microsoft Forms example and my SharePoint list for detail. Thank you.

When you say forms, are you referring to Microsoft Forms: https://forms.office.com/? If not, you'll need to further explain your form and how that is integrated into your workflow. If you are referring to MS Forms, once you set up the form, the questions are fixed. How are you changing them? It would be best to show your flow with your question, so people have a better understanding of what you are trying to accomplish.

Hi, @David MA Thank you for responding. My answers are being populated properly, some of the questions are a couple of sentences and vary from various Stakeholders, so I want my sort and reports on the Stakeholder. I don't know if a FX function would work to pull the questions into the SP list, I'm at a loss and stand still. The annual questions vary from Stakeholder to Stakeholder and need a history when sorted.

If I place the question in the list, then I need to copy and paste it into the for, just trying to minimize those extra steps. The A1 and Q1 etc column names are just reference, they are actually Question 1 etc... Answer 1 etc...

Once you have your SharePoint list set up to store the corresponding responses from your MS Form, the rest is quite easy. Create your workflow with the trigger When a new response is submitted. Follow it with Get response details and set the ID to the ID in the trigger. Then add a Create item in SharePoint action. For the questions, you will need to manually type those in based on the questions in the form, but the answers will come from the Get response details action. That's all you need to do.

Although, to me, it would make more sense to just populate the answers and don't bother populating the list with the questions. You can either name the fields for the answers with a shortened version of the question, or keep it like you have with A1, A2 as that will make it easy in the workflow and set the question as the description. This way, when you open the SharePoint list item, it will show as:
