Hi,
I am using trying to use the Send a survey when a case is resolved in Dynamics 365 template. I have configured it and the flow is successfully geting triggered however when it is attempting to send the send the Flow survey to the recipient I get a the following error:
Action 'This_action_sends_a_survey_to_a_specified_list_of_recipients_2' failed
Error details: Resource not found
Any idea how to solve this? My guess is that it is the email template that it can't find but how can I check this and work out why? Below is a link to an image of the error.

Thanks for your continued help - I have tried all your suggestings and they unfortunatly have not effect. I have had confirmation from Microsoft Support team this morning that this is an known issue that effects some people and the product team are working on a solution.

Can you confirm the access to Forms Pro? Did you try the export and import part I suggested in one of the replies. Also, did you get a chance to implement the caching suggestion I pointed in the earlier resposnes. These type of errors/ issues usually come up due to caching issues and you might wnat to have a look at that.

Thanks for the help - unfortunatly it is still not working, something to do with the connection I guess. I checked and the survey is still there (I can select that fine) it is just the email template that I can't select.
I have an open support case with Microsoft so hopefully that will work it out.

I have tested it on my side, and it works well.
The attributes of the Email template need to be used while sending the invitation. You must select an email template from the list and not enter a custom value.

And your issue shows that the Email template has disappeared, could you confirm the Survey you owned has not deleted, and create a new connection, after that, re-start your flow.

I have faced this issue before and this worked out for me:
Two things here:
1. The send survey action is for the Forms Pro connector (does your organisation have a subscription to the Forms Pro service?) If not, please go to forms pro and click on sign in and if the forms pro is enabled for the organisation, it will connect you immediately and then you will see Surveys etc.
2. If you already have done 1 then this might be a caching issue. I would suggest you to delete the current connection to forms and forms pro and create new connections. Log out of the current session and close the browser window. Start a fresh session and check if you are facing the same issue.
You might want to export the flow as .zip package and import that back and provide the connection during import.
Also, you can create a simple test flow and then in that use th manual button trigger and then add the action send survey and check if you face the same issues.

Thanks for your reply. Here is screen shot of the flow:
When I created it from the template I had issues with selecting the email template - the drop down list just says Loading then 'Could not retrieve values. undefined'. The only way I could get it to work was by creating the flow from Forms Pro and it prefilled everything (I have opened a support case about this).
I guess this is the cause of the problem? If I goto edit the flow and remove the tempate Guid I get the same issue now:
Any ideas? I am the owner of the form.

Can you please post a screenshot of your flow?
Ensure that you are selecting the form and the email template properly from the drop down.
Also, you have to be the owner of that form/survey. If it is shared with someone, you still have to be the ower of that. And this would not work on a form moved to a group.
