How can I set an automated reminder in Sharepoint List based on a date field + 6 weeks and 12 weeks?

I have a list of appointments which I want to set a reminder for their follow-up appointments at 6 weeks and 12 weeks. I have manually entered the date fields for the follow-up appointments, is there an easier automated way to do this and subsequently set reminders for each of the line items for both 6 and 12 weeks?

You might try this:

Create a Flow with a recurrence trigger set to run once a day

Follow the trigger with a SharePoint Get items action

Create a filter query so that only items at the 6 week mark are returned

Loop through the items and send an email.

Do the same thing for the 12 week reminder.
