I tried a different approach and got to the point of creating a flow but it doesn't see the form created in PowerPages.

A little progress. In one environment I still get the blank popup. In a different environment, I was able to create a flow. However, it has to be an instant cloud flow not an automated cloud flow. While I now have a flow in the solution and associated with the site, the form's on submit still does not recognize the flow. See: PowerPages form missing option to submit to a Powe... - Power Platform Community (microsoft.com)

To add an automated flow to Power Pages, you need to1234:

    1. Sign into Power Pages.
    2. Create a new solution or select an existing solution.
    3. Select +New, Automation, Cloud flow, and then Instant.
    4. Select Skip.
    5. Search for Power Pages Select When Power Pages calls a flow trigger.
    6. Define your flow steps and return values and select Save.
    7. After saving the flow, add it to the site and assign a proper web role.
    8. Create a site with one of the starter layouts.
    9. Choose the site and select Edit.
    10. Navigate to the Set up workspace.
    11. Under App integrations, select Cloud flows (preview).
    12. Select + Add cloud flow.

FYI: the flow connector you have selected for your Flow is not correct there is a specific one for Power Pages (the one your screen shot shows is for Microsoft Forms - a completely different product).

One scenario (may not be your scenario) where you can get the blank window when trying to create a Flow is that your user is not licenced for Flow.   There is a direct url for Flow (that I can't find at the moment) and if you use it it will tell you if it is a licencing issue.

I have created the cloud flows also they are working properly but for integration with power pages when I reach to set up navigation after completing all steps it shows flows in other flows section there, I try to add flow to this site after adding roles on right side pop up window it displays error on top below preview bar that this flow cannot be added to the site this is happening with all my flows when I try to add them to the site.
