Dataverse design of a pre-operation checklist

I am trying to design a collection of tables that store information related to a daily checklist that is completed before an operator can use a construction vehicle.

Each checklist question comes with a hazard scale from 'A' to 'C' ranging from needing immediate repair to being repaired within a week, respectively.

The certain part being checked for only needs to be checked if it is marked as 'F' (faulty) on the checklist, otherwise it is marked as okay. If it is faulty the foreman should know exactly which part is faulty (i.e. get a list of faulty parts on the vehicle), this should replace my own basic system of maintaining an overall required score which if not reached will prompt the foreman to review each answer one by one.

The design for this has been racking my brain for quite sometime, your most urgent help would be appreciated.

    It will be good, if we go with Canvas PowerApps for Building the same.
