I'm looking for solution which allows me to populate DocuSign template from form filled before.
The system should be like this:
Person open 1st time or again form and update it - trigger
That creates document with personal data from the form.
It's possible?

Yes. That can be done. You can use the when an item is created or modified trigger for the flow then. Everytime somebody creates an item in SharePoint list or updates a previous one, the flow gets triggered and then you can populate the template and continue with further actions in the flow.

SharePoint list can be done like editable form? It have to populate from database maybe based on email matching or whatever and it have to be editable.

yes that is doable. However, it will work with MS forms only for new submissions. As a form once submitted cannot be edited so the details cannot be updated.
You need to add the appropriate fields to docu sign template and then use the trigger when a response is submitted and then get the response details, populate the docusign template and perform the consecutive actions.
