Model Driven App strange issue with dates that change without reason

Hi,

I have been pulling my hair out by looking at this issue our customer has with their app.

The following situation should not happen and I cannot find the reason why:
Two weeks ago our customer found out that some dates have been changed to the current date. Even if the field is locked and will be set by a business rule.
Even inactive records have dates changing to the current date.
 
Background information of the app:
Our customer has a model driven app where they register incidents, complaints and how they solved the problem by reasearching what happened.
In the model driven app we have a custom table, with four or five date fields and a choice field for active stage. We use the standard datetime field set to date only. 

There are a few Power Automate flows to send emails as reminders and a single Power Automate to add the 'relationship' between the table and a Business Process Flow. 
The Business Process Flow has multiple stages: Called In, Registration, Research, Check and Implement.

The business rule to set dates checks if the stage is equal to the active stage and only then set the date to today.
Even after disabling the rule the date still changes or is set when the stage changes.

What I tried:
- Disabled all flows, except the one needed for setting the relationship between the record and the BPF.
- Disabled all business rules.
- Checked other solutions
- Checked the BPF for anything related
- Checked if there are scripts, assemblies or plug-ins
- Checked if there are scheduled system jobs
- Checked audit log (unfortunately the customer didnt have logging enabled)
- Checked for recent changes in the environment.
- Checked other environments
- Checked for unmanaged layers
- Also happens on inactive records
 
Up until two weeks ago it worked fine and suddenly dates are changing.
It happens on multiple environments and the solution has not been updated in a year as it worked perfectly.

Does anyone know or give me some pointers where else I could check?
If screenshots are needed I will make some and make customer data anonymous

    Hi Mike,

    I have asked my collegue and it appears to be resolved on its own. They cannot get in reproduced anymore.
    It was so strange as it happened on all environments three weeks ago, so probably an issue my Microsoft as it works for now.
    I will accept your answer.

    Hi Mike,

    I have been a few days off from work and a collegue who has more Model Driven App experience has taken over this issue.
    I will ask him if he solved the issue or that it still persist when I am at the office later today.

    So the only thing I can think of is, someone has a copy of the Flow running. Whether in a Dev environment, or person environment and its triggering as its pointing to the wrong place.
     
    Or, there are some that are for some reason still running (instances) so make sure to go to the flows and verify under history that none are running
     
    or someone has an instance in Test mode.
     
    or some massive retries are happening.
     
    I'd have to review your entire solution etc to see whats up.
    I would love to see the Flows, where they are triggered and making changes, even though they are off.
