What’s happening?
Quick create forms in field services stopped working suddenly, affecting the ability to create new records (such as contacts and vendors). 
 
Reason:
These entities had been removed from the Field Service app, which prevented the quick create forms from functioning across all entities.
 
Resolution:
Ensure the entities have been included in the model driven app being used.
