How to dispaly banner/notification at the top in model driven app ?
If one choice is YES
Then Banner should always display and user will not have a way to hide or close the banner message.
how to handle the same ? Pls help

In this scenario, you can use JavaScript that will work fine for you.
There can be 2 events to show banner message
1. OnLoad -> This is when you open the Record Form
2. OnChange -> Will fire the JS when your choice field is modified.
