I have a data table with 12 rows and 5 columns

if your table has 5 columns then you can’t have column number 5, because it’s 0 based numbering (e.g. column 5 is number 4). I think you will then also need to use %LoopIndex-1% for the row index.
I see problem is that column numbers are 0 based
